VIA POSTAL MAIL
The cost is $20.00 each. Orders are shipped within 2 weeks by U.S. Postal Service First Class Mail. You will receive a standard wallet-size plastic card. The card will be our current design, with our current logo, and will include your name, school and the year you passed the National Board Exam.

The International Conference accepts only money orders, cashier's checks or major credit card. To order, please download and print the form, enclose appropriate payment and mail the completed order to:
